I read an AAR where all of the production cities of Poland are taken in 2 turns (Warsaw, Lodz, Krakow, Poznan) and Poland surrenders. However I've done the same in a new game and Poland hasn't surrendered. On the war panel morale for Poland shows 11. Do I have to knock it to 0? Is it just random die roll and luck that they haven't surrendered already? Other than capturing all production cities what do I need to do to make them surrender? Just kill units? Something else?

A few possibilities:
You haven't ended the turn. Surrender does not occur at the moment you occupy the conditions only when you end the turn. So unless you want to earn experience at the risk of losing steps just stop attcking once you've captured the objectives and start moving units west.

You didn't actually occupy the cities only killed the units therein. Easy to miss at times. An opponent forgot to occupy Paris on me once ... but he still kicked my butt in USSR

Or this is a bug and needs investigating.
