Does the 4th Pz HQ arrive as a reinforcement in Trappenjagd or does it have to be built?

You have to build all HQs.

I don't even build it. I just take the 11th Army and rename it 11th Panzer army and add my incoming panzer divisions to it. Mainstein is already the army commander so in that respect you are ready to go.
