There are two columns because one describes the element's state at the beginning of that round of combat, and one describes its state at the end of the round of combat. If the first column is empty and the second says DEAD, then that element began that round alive and ended it dead. If both columns say DEAD, then that element began (and ended) that round dead. RTR means retreat (retreated, retreating), as far as I can tell. Yes, the manual doesn't answer every question like this, but I was able to figure these out on my own. I agree that it's a vast overstatement to call the manual a "disaster." I learned the game mechanics perfectly well from the manual, even though I'd never played DB 1.
