As for question b, we're talking about 1 point subtracted for every 10% roughly under TOE. So if a HQ calls for 250 support squads in it's TOE, for every 25 missing squads you will suffer the 1 point penalty (I don't think it's exact, may be some random in the 25). This is different than support need.
Support need is different from the unit's percent of TOE strength. To be at 100% of TOE strength the HQ must have all 250 support squads (in example above), but it will never suffer a penalty in the logistics phase for not having enough support squads for it's support need since only non-support squads require support (and HQ's have no non-support squads). Now in hindsight it might be nice to have fuel and supply dumps in HQ's require a certain amount of support, but that isn't in the game at the moment AFAIK, (they do require vehicles to move).
I don't know the answer to question a, but you might find it in the manual if there is an example of leader checks.
