Do I need to use the update from the start up screen or can I just use the Patch and be good?

I think the start up screen "update" just takes you to the Matrix location for the latest "Official Patch" and you have to download it and install it over your existing version.

After that you can download and install Michael M "Beta Patches" that fix/modify various issues but have not been officially approved/adopted by Matrix yet.

Save your "save game" files before updating in case something goes wrong. I assume you have your CD from your initial install? You will need the serial #.

If you run into any difficulty downloading, just send a request (with game serial #) to Matrix support and they will set up a download link for you.
